Hometown Health Plan (HHP) Pre-Bariatric Surgery Program

The Center for Nutrition and Metabolism is pleased to assist HHP clients with the weight loss and nutrition assessment requirements preceding bariatric surgery through Western Bariatric Institute.  This mandatory, pre-bariatric surgery program will give you a chance to assess your commitment to the dramatic dietary changes necessary after surgery.  In addition, this program will also support you in losing 10% of your initial body weight, recommended prior to surgery. 

After completing two initial assessment visits with our Physician and a Registered Dietitian, HHP clients have the opportunity to join one of two 12-week program options described below:

Programs:

HMR Healthy Solutions® Meal Replacement Program

Physician Directed Program

Length:

13 weekly group classes, 1.5 hours in length with a trained HMR® program Health Educator. 

13 weekly individualized appointments, varied in length with interdisciplinary providers – determined based on needs.

Specifics:

Compliance with weekly group classes and program commitments and imperatives - using meal replacements for the full 12 week period, including: entrees, shakes, bars, soup, multigrain hot cereal and unlimited fruits and vegetables.  HHP does not cover the cost of the meal replacements.  Attendance at a free program orientation is required.  This is a highly structured, convenient and decision-free program. 

Compliance with weekly appointments and provider recommendations, consistent and regular tracking of daily food intake using a nutrition software program and regular use of a pedometer for self-monitoring of physical activity for the full 12-week period. 

Results:

Average weight loss is 36.7 pounds over the 12-week period.

Average weight loss is 10-25 pounds over the 12-week period.
